Assessments in Office

Please download the forms using the links below. Complete them, then email them back to us at allstarccllc@gmail.com. You may drop them off to us or bring them in with you completed for your visit.

You will also need to send or bring with these documents a valid ID, Social Security Card (for the client and parent if the client is a minor) along with the client's updated insurance card, and if you have a covid vaccine card, please bring this as well.

Someone will contact you to confirm your appointment within 24-48hr business hours for either your office assessment.

Please note that if you need immediate attention please feel free to call us at the office to schedule your appointment at 225-923-3733 during regular business hours.
